HIV Case Aide

Position HIV Case Aide  
Description

A new grant year in our Ryan White program offering the opportunity for a new Case Aide to help clients complete the Ohio HIV Drug Assistance Program (OHDAP) application and ensure completion and submission to the Ohio Department of Health (ODH).  The Case Aide will also assist with data entry, outcome and disbursement in the database, and distribution and collection of ODH required surveys.

Responsibilities:

  • Schedule and assist with client appointments, ensuring proper documentation and follow-up.
  • Assist clients with accessing medical, social, and support services, including transportation, housing, and food assistance.
  • Maintain accurate client records in compliance with program regulations and confidentiality guidelines.
  • Provide support to the Case Manager in client assessments, care planning, and ongoing monitoring.
  • Act as a liaison between clients and service providers to ensure timely delivery of services.
  • Educate clients about available resources, treatment adherence, and health maintenance.
  • Perform outreach to potential clients and community partners.
 
Position Requirements
  • Associate’s/bachelor’s degree required.
  • Prior experience in case management, social services, or healthcare is a plus.
  • Knowledge of HIV/AIDS, community resources, and health services is preferred.
  • Strong communication, organizational, and interpersonal skills.
  • Ability to work collaboratively within a multidisciplinary team.
  • Valid Ohio driver’s license and insurance required.
