The role is fast-paced and primarily focused on discharge planning with older adults, assessments, and medical record documentation. Key Responsibilities: Conduct assessments and develop individualized social service plans. Facilitate discharge planning and coordinate post-care needs. Maintain accurate medical record documentation. Support residents and families in adjusting to a long-term care setting. Ensure compliance with resident rights and assist with Medicaid applications. Collaborate with the interdisciplinary team on resident care planning.     Position Requirements Requirements: Recent rehab experience. Flu shot required (COVID vaccine not required).
