Personal Service Coordinator - TAY 


Job Summary:

The Personal Service Coordinator is responsible for clinical care and case management of assigned youth and families in a community, home, or school setting.  This includes assisting clients with mental health, housing, vocational, and educational needs.  The position reports to the Team Leader and is also responsible for case management, administrative documentation, and participating in trainings as assigned. Along with the rest of the team, the Personal Service Coordinator is active in supporting and implementing wraparound philosophy and its individualized, strengths-based, culturally competent, family-centered approach.   

Duties and Responsibilities:


Direct Care and Interventions: 1) Provides scheduled individual and family therapy for program participants and their families, primarily in the field but sometimes in the office; 2) provides clinical leadership to program team; promotes team building and problem solving; 3) leads development of Youth Partnership Team (YPT) consisting of program staff, youth, parents and caregivers (if applicable), child welfare worker or other agency staff, Family Partner, Peer Mentor, and other significant people in client’s life;  4) Organizes Youth Partnership Team meetings including contacting team members as needed to schedule team meetings; 5)  With support from the Team Leader as needed, facilitates Youth Partnership Team meetings for program participants and their families, utilizing wraparound principles to assist the team decision-making process; 6) focuses on youth and family needs and strengths, care plans and safety plans; 7) Assesses needs and works with YPT to locate and arrange appropriate long-term housing or placements of clients as needed including living arrangements, educational and vocational placements, and participation in other community services; 8) participates in Individual Educational Plan and other school meetings whenever possible; 9) encourages and supports family participation in these meetings.

Case Management: 1) Collaborates with community resources including county workers, courts, professionals and other community resources, vocational development and independent living skills programs, schools, colleges, housing providers, parents and significant others; 2) keeps key team members informed and communicating; 3) works collaboratively with other team staff to assess client/family needs and to identify appropriate natural supports and community services to meet those needs.

Crisis Care:  1) Participates on a rotating basis in providing on-call coverage in accordance with agency policy; 2) Provides crisis counseling as needed to assigned youth/families.

ADMINISTRATIVE:  (30% of job)

1) Completes treatment plans, mental-health progress notes, quarterly reports and other documentation in a timely manner; 2) ensures that all needed consents and other paperwork is completed for each client at the time of entry to the program and ongoing; 3) participates in Utilization Review meetings at County office as needed to authorize treatment; 4) participates in agency committees as assigned; 5) provides on-going orientation and training to staff as requested; 6) May provide supervision of Peer Mentors, Family Partners, or other program staff.

Position Requirements


  1. MSW or MA in Counseling from an accredited college or university.
  2. Registered intern with the Board of Behavioral Sciences, license preferred.
  3. At least one year of post-graduate residential, community-based or milieu experience preferred.  Experience with TAY strongly preferred.
  4. Fluency in prevailing language(s) of community where Personal Service Coordinator will work preferred.
  5. MUST HAVE expertise in at least one of the following areas: housing, employment, or education for youth, or substance-abuse recovery.
