The Dialysis Social Worker provides critical support to clients, addressing social, emotional, and mental health needs while adhering to healthcare and behavioral health regulations. This role involves conducting biopsychosocial assessments, developing individualized care plans, providing therapeutic interventions, and coordinating with healthcare providers and community resources to ensure comprehensive client care.
