The RN will lead and conduct care coordination/management with internal and external entities and collaborate with behavioral health and primary care staff internal and external to Southeast.   Position Requirements Ohio RN license required. Must have strong knowledge of chronic conditions and experience using data to achieve outcomes. As a healthcare provider, Southeast Healthcare is a non-smoking employer.
