Care Coordinator needed for part-time (30 hours/week), temporary (6 month) position.

The Care Coordinator will support members living primarily in the Center City of Philadelphia, and parts of Montgomery and Delaware counties. This hybrid position will run from February through July of 2025, working from a home office and in the field with adults aged 60+ under the direction of the Director of Care Coordination.

The Care Coordinator will support members designated as Well, At-Risk, and In-Care. Using the applicable member contact schedule, the Care Coordinator will maintain regular contact with assigned members and/or responsible parties. This includes videoconferencing and in-person visits, utilizing clinical screening tools and assessments to determine members' eligibility for preventative initiatives, community resources, and/or other services, including Medicare or services covered under the organization.

Key Responsibilities:

  • Maintain contact with members and responsible parties, using clinical tools to assess needs.
  • Conduct video and in-person visits as required.
  • Coordinate services and recommend preventative initiatives and resources.
  • Participate in educational planning and member meetings.

 
Position Requirements

BSW or MSW with a minimum of 3 years related healthcare experience. Reliable car for travel to in-person visits. Strong problem-solving and crisis intervention skills. Excellent organizational and time management abilities. Proficiency or willingness to learn Microsoft Office programs.
