Medical Social Worker needed for full-time, temporary (11/18-12/30) position in Columbia, Maryland. This is a 9a-5p position.

This is an excellent opportunity for someone looking to provide impactful care in an established organization.

This role will involve providing psychosocial assessments, counseling, discharge planning, and resource coordination for individuals receiving care in our facility. The ideal candidate will have experience working with aging populations and those facing chronic health issues, offering both emotional support and practical assistance.

Responsibilities:

  • Conduct initial psychosocial assessments for new patients to identify emotional, social, and environmental needs.
  • Collaborate with healthcare teams, including nursing and therapy staff, to develop comprehensive, patient-centered care plans.
  • Provide short-term counseling and crisis intervention to patients and their families, addressing issues related to illness, adjustment, and coping.
  • Assist in discharge planning, coordinating services for a smooth transition back home or to other care settings.
  • Connect patients and families with community resources, such as housing support, financial assistance, and mental health services.
  • Document all interactions and interventions in compliance with facility standards and state/federal regulations.
  • Act as a liaison between patients, families, and medical teams to ensure clear communication and holistic care.
  • Educate patients and families on topics like advance directives, coping strategies, and community resources.
 
Position Requirements
  • Master’s degree in Social Work (MSW) from an accredited program; clinical licensure (LMSW, LICSW)
  • Understanding of the psychosocial aspects of aging, illness, and care transitions.
