Description

HIV Medical Case Manager needed for full-time, temporary position in Philadelphia.

Provide comprehensive case management services to clients, including intake per Uniform Access procedures, face-to-face needs assessments, and gathering of medical, demographic, and consent information. Develop care plans collaboratively with clients and support teams, maintaining confidentiality.

Manage a caseload of clients with HIV, providing advocacy and support through home, hospital, clinic, and office visits as needed. Offer case management at Philadelphia Health Center #2 for half to one day per week and support tenants of subsidized housing. Assist clients in accessing social services and ensure connections to other systems. Provide information, referrals, follow-up, and reassessments as needed, along with education to clients' support systems with consent. Participate in discharge planning, offer intake coverage, and perform other duties as required.

 
Position Requirements

BSW or MSW, expertise in case management and advocacy, ability to work with diverse populations in critical situations, sensitivity to cultural, economic, gender, and sexual orientation differences, bilingual (Spanish/English), and understanding of medical and psychosocial aspects of HIV and related conditions.
