Pay Range: Starts at $60,373.00/year. Based on your education, experience, and credentials.
Available Work Schedule: (Full-Time)
- Monday - Friday: 8:30 a.m. - 5:00 p.m.
What You Will be Doing
We want people to join our team who share our values of love, excellence, respect, empowerment, and well-being. The ideal candidate will be responsible for supervising and managing Care Coordination staff providing care coordination and behavioral health services for OhioRISE, I Am Boundless. Working with clients, parents/guardians, staff, and community partners, the Care Coordination Manager facilitates access to care and addresses barriers. The Manager will provide support, care coordination, and linkage with outside providers to promote meaningful treatment and improve outcomes for the individuals served. This position reports to the CME Director and works closely with all catchment area providers, Aetna and Ohio Department of Medicaid (ODM). This is a full-time, salaried hybrid position.
Required Education and Experience:
- Master’s Degree in Social Work or related field of study.
- Three years of experience in children’s behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral healthcare field, providing community-based services to children and youth and their family or caregivers.
- Three years of supervisory or management experience.
- Experience working in the behavioral health field.
Preferred Licensure/Certification:
- Current Ohio mental health professional license including LISW, LPCC, LIMFT, PSY, LIMFT-S, LISW-S, or LPCC-S.

Must be able to function independently and have flexibility, personal integrity and the ability to work effectively with residents, staff and support agencies.

Must be able to relate to and work with ill, disabled, emotionally upset and at times hostile people within the programs.

Must possess the ability to make independent decisions when circumstances warrant such action.

Valid Ohio Driver’s License with Ohio Bureau of Motor Vehicles - No more than 5 points on driving record.

Valid car insurance.
