The Family Case Manager connects facility residents and their children to external resources for securing permanent housing and support. Responsibilities include individual case management, advocacy, needs assessment, action planning, and coordinating group meetings for emotional support. They accompany residents to appointments, provide informal counseling, and refer to external services when needed. This role also involves networking with community resources on topics like women's health, housing, and domestic violence.   Position Requirements BSW with direct service experience is required; residential setting and urban experience are preferred.
