Job Title: Home Health Physical Therapist (PT)

Status: Part-time and PRN opportunities available, extremely flexible

About the Role:
Community Physical Therapy (CPT) is seeking an experienced Home Health Physical Therapist (PT) to provide services in Assisted and Independent Living communities in the Aurora and Warrenville areas. This position requires recent experience with OASIS SOCs and a passion for geriatric rehabilitation.

In this role, you'll deliver home care services directly to residents in their living environments, and there may also be opportunities to provide outpatient services as part of CPT's Continuum of Care program. This is an excellent opportunity for therapists who enjoy working as part of a dedicated team to provide skilled care in the geriatric community.

Qualifications:

Recent Home Care experience is essential.

Current experience with OASIS Start of Care (SOC) assessments is required.

Passion for working with geriatric populations.

Ability to work collaboratively with a multidisciplinary team.
