SUMMARY

Under the direction of department managers the Lead Direct Support Professional will assist with the development and implementation of individual support plans as part of a day habilitation and prevocational program placement for individuals with intellectual disabilities. The Lead role will aid in educating, coaching, and training Direct Support professionals on efftective care techniques

ESSENTIAL DUTIES AND RESPONSIBILITIES

  1. Utilize appropriate methods for working with individuals with intellectual and or physical disabilities.
  2. Report suspected Abuse, Neglect, Exploitation, Isolation, and Abandonment (ANEIA) within 1-hour as mandated by state law.
  3. Provide personal care support for individuals served and the daily coordination of Direct Support Professionals.
  4. Provide department specific training/mentoring for all new team members and continuing , training/mentoring for current team members.
  5. Conduct daily team huddles to solicit feedback, answer questions, and communicate relevant information to Direct Support Professionals.
  6. Provide managers with information regarding team member performance.
  7. Read service and habilitation plans for all individuals to ensure proper care according to their plan(s).
  8. Assist individuals toward the achievement of their individual goals and to perform and participate in daily scheduled activities and collaborate with Career Opportunity Advocate as needed.
  9. Ensure proper documentation is complete (i.e. ID notes, communication logs, schedules, incident reports and Paycom).
  10. Responsible for maintaining records (i.e. monthly calendar, daily activities, active treatment, scheduling, community outings and reporting volunteer hours).
  11. Maintain the compliance of team member to individual ratio.
  12. Maintain clean and safe work environment.
  13. Anticipate safety needs for all individuals and team members and report any and all issues or concerns to department managers as soon as possible.
  14. Comply with all Federal, State (Desert Regional Center - DRC) and local regulations and requirements, and company policies and procedures.
  15. Submit driving logs, reporting any issues or concerns to managers regarding vehicle keys and cell phone for outings.
  16. Attend Person Centered Plan (PCP) meetings.
  17. May be assigned other duties as needed.

SUPERVISORY RESPONSIBILITIES

With the support and assistance of the department Supervisor,this position coordinates and provides team leadership for Direct Support Professionals.

 
Position Requirements

SKILLS REQUIRED

    • Ability to communicate effectively with individuals, team members, and supervisors (verbal and written).
    • Ability to care, respond, and work with individuals with developmental disabilities
    • Must encompass professional demeanor.
    • Ability to define problems and draw valid conclusions.
    • Ability to interpret instructions, gain knowledge and have initiative.
    • Computer literate.
    • Ability to complete required documents, i.e., ID Notes and Incident Reports.
    • Must possess social perceptiveness to assess and understand other’s reactions and behaviors.
    • Ability to resolve multiple complex issues simultaneously while remaining calm in stressful situations.

EDUCATION AND EXPERIENCE

  • Required: High School Diploma or equivalent.
  • Required: Two (2) years verifiable work experience as direct support/care staff in related field, i.e., health care, adult day care, mental health or special education.
  • Required: Current CPR and First Aid Certification within 30 days of employment
  • Required: Current CPI certification within 30 days of employment
  • Preferred: Experience working with individuals with disabilities.
