The Case Manager develops a person-centered Integrated Family Case Plan that promotes recovery, connects individuals to appropriate care and community resources, and facilitates skill assessments. They refer clients to substance use disorder treatment as needed, ensuring weekly visits of at least one hour are offered. The Case Manager coordinates interventions across multiple agencies, assists with transitions to other care levels, and uses engagement skills to motivate clients to access services for a healthier lifestyle. They advocate for clients, provide education on HIV and Hepatitis, train on relapse prevention, and help avoid relapse.   Position Requirements BSW or MSW preferred, with three years' experience in substance use disorders.
