Community First Health Centers is currently looking for 2 Quality Data Support Specialists to join our team at our New Haven medical facility. Under the supervision and oversight of the Quality Director, the position is responsible for maintenance of the outreach and quality reporting systems and data points including uniform data system measures, patient outreach and other quality related reporting systems. The ideal candidate will be detail oriented, motivated and a team player.

Daily responsibilities of the Quality Data Support Specalist may include:

  • Promotes and maintains awareness of Agency programs, services, and activities
  • Responsible for acting as CFHC’s on-site resource for all data entry components including uniform data system measures and report elements, patient satisfaction, outreach, and quality reporting
  • Primary support contact for outreach and quality data reporting system with responsibility to investigate and problem solve/resolve related issues and discrepancies
  • Communicate to Quality Director any issues, updates, or changes needed as related to the outreach and quality data reporting systems including any needed changes in settings or workflows.
  • Promote and maintain awareness of Agency programs, services, and activities.
  • Serve as CFHC’s on-site resource for all data entry and reporting programs/systems, including uniform data system (UDS) data and measures, patient satisfaction, outreach, and quality reporting.
  • Act as the primary support contact for outreach and quality data reporting systems, with responsibility for investigating, problem-solving, and resolving related issues and discrepancies.
  • Communicate issues, updates, or necessary changes related to outreach and quality data reporting systems to the Quality Director, including adjustments in system settings or workflows.
  • Accurate and timely entry of necessary data into the electronic medical record (EMR), electronic dental record (EDR), outreach systems, or reporting software into various programs used for quality, risk, or compliance purposes.
  • Create and update dashboards, generate reports, and conduct data validation according to an established task assignment, reporting, and data validation calendar.
  • Design and present visual reports and dashboards tailored to specific audiences, effectively communicating relevant data.
  • Maintain close working relationships with medical records, the EMR administrator, and clinical team members to ensure accurate capture of required data for UDS, PCMH, Azara, value-based care, and HEDIS.
  • Collaborate with Practice/Program Managers, Quality Department, and EMR Specialist on EMR/data issues.
  • Create and distribute patient outreach materials for those in need of services via print, electronic, and/or verbal communication, based on patient populations identified by compliance and the PI committee, ensuring alignment with HEDIS, UDS, and other quality measures set by insurers and accrediting organizations (HRSA, FTCA, NCQA, PCMH, etc.).
  • Assist with payer gaps in care and other health plan record requests or site reviews under the guidance of the Quality Director.
  • Attending meetings, seminars, workshops, and conferences for professional development and representing CFHC as needed.
  • Seek opportunities to improve internal satisfaction and increase efficiency in work tasks, data presentation, and overall work product.
  • Distribute QA Medical Record and Professional Staff Peer Review forms as directed by the Quality Director.
  • Assist in copying and assembling meeting or educational packets and materials.
  • Plan and complete job duties with minimal supervision, meeting deadlines and due dates as assigned.
  • Travel to other Agency locations as required or directed.
  • Support other Quality-related and Performance Improvement Initiatives as directed by the Quality Director.
  • Perform other duties as assigned to contribute to the overall success of the Agency.

This is a full-time, 40 hours per week position.

Hours will be gained Monday through Friday, 8:30 am to 5:00 pm.

Position Requirements

Education and/or Experience

  • High School diploma or equivalent required, associate degree or equivalent experience in health care preferred.
  • Demonstrated proficiency in computer skills and functionality.
  • Demonstrated proficiency or formal trianing in Excel spreadsheets and data visualization reporting.
  • Knowledge of medical terminology preferred
  • Detail oriented with a high level of accuracy
  • Flexible with the ability to work independently and with minimal guidance
  • Strong oral and written communication skills
  • Excellent interpersonal and customer service skills
  • Must maintain Basic Life Support certification
