Skill Creations is a human services agency that operates three service divisions. The ICF Division provides specialized long-term residential care to 191 individuals with intellectual and/or developmental disabilities in 15 different towns in North Carolina. The Community Operations Division provides residential and non-residential services to persons with intellectual and/or developmental disabilities in their home community. The Child Development Division, with two locations in Goldsboro, provides high quality child care and preschool for children ages birth - 5 years. One center also offers after school care through age 12.   Description The Habilitation Coordinator position is responsible for the supervision of 3 shifts of direct support staff that serve adult individuals with physical and/or intellectual disabilities and /or medical care needs residing in a 24 hour group home. This position must assure adequate staffing on a daily basis. The position assists in establishing programs within the home, inclusive of monitoring program implementation, providing hands-on mentoring with regards to programming and training direct support staff to carry out established goals. This position will monitor and ensure that direct support staff are following guidelines to meet residents' individual needs which includes but is not limited to: goal training, data collection, personal care, home-living tasks, leisure activities, and etc. Individual will be trained in usage of an electronic medical records system that will be used for data collection, etc. Individual will be responsible for supervising and training staff in it’s usage. The Habilitation Coordinator may also act as medication monitor and in an emergency, back up in the kitchen. This position would also serve as on-call staff for the facility on a rotating basis. Starting salary: 45,000 annual     Position Requirements Requirements: High school diploma or equivalent. Additional training and/or college level course work preferred. Valid Driver's license with clean driving record (for insurance purposes).  Working knowledge of office equipment including copiers, fax machines, and computers. Basic Proficiency in MS Office (Excel, Word). Ability to pass various training courses, both online and in person, including CPR/1st Aid and CPPI. Excellent time management skills, the ability to work independently, and prioritize work. Excellent customer service skills. Problem solving skills and attention to detail. Excellent written and verbal communication skills.
