Both per visit and full-time openings are available. Hourly compensation plus mileage is included. Social worker will be responsible for: Developing a comprehensive assessment and care plan with the patient and family members; Assisting with community resources; Provide counseling and supportive services to patient and family members; Collaborating and communicating with all members of the treatment team; Documenting all activities in the electronic medical record.   Position Requirements Candidates must have a master’s degree in social work from an accredited school of social work and be licensed as a LMSW or LSCSW in Kansas.
