Community First Health Centers is currently looking for an exceptional Behavioral Health Clinician to join our team!!

The ideal candidate for this role will be enthusiastic, motivated and a team player.We currently have a Behavioral Health Specialist; MSW-Clinical opportunity available at our medical facility under our Homeless Healthcare & Behavioral Health service line. The ideal candidate for this role will be dedicated, detailed and a team player. Under the supervision of the Homeless Healthcare & Behavioral Health Program Manager, you'll provide behavioral health consultations in the primary care setting to children, adolescents, adults and families Under the supervision of the Behavioral Health Program Manager, you'll provide behavioral health assessments and counseling in the primary care setting to children, adolescents, adults and families to improve psychosocial functioning.


Daily responsibilities of our Behavioral Health Clinician may include:

  • Complete and maintain all required data and documentation of services to clients for both case and Agency records including referrals, intake, treatment plan(s), progress note(s), status reports(s), case management, discharge and follow up services in a timely manner
  • Performs initial assessment and monitoring of a patient’s social, emotional, behavioral, environmental, financial and support needs
  • Provides behavioral health care and collaboration with an interdisciplinary team including primary care, dental, psychiatric, etc.
  • Provides consultation and collaboration with other members of the treatment team informing of all pertinent factors that may affect the patients’ overall health and well being
  • Develops a client driven plan of care that addresses the clients’ needs with mutually agreed upon goals that are measureable and time specific
  • Works with the patients’ family, other service provider(s) and/or other individuals deemed important by the patient to develop a plan of care at the located deemed most convenient for the client
  • Perform advocacy for the patient and at times be an advocate for broader social cause(s)
  • Submit timely and accurate client daily activity logs/encounter forms for billing/statistical purposes
  • Provide individual and/or group therapy and/or crisis intervention as needed
  • Provides education on issues of behavioral health to service recipients, families, members of the community and other staff

Position Requirements
  • Masters Degree in Social Work with a Clinical Speciality
  • State of Michigan licensure as a LMSW- clinical , LLMSW- Clinical or Licensed Professional Counselor (LPC)
  • Experience in working with medical health recipients and persons with behavioral health issues
  • Valid State of Michigan Driver’s License in good standing
  • Reliable transportation with current auto insurance
  • Experience with an EMR system with above average computer proficiency
  • Must maintain Basic Life Support certification
