Intake and Assessment Specialist needed for full-time, temp to hire position in Philadelphia County.

The Intake & Assessment Specialist is responsible for managing referrals and conducting face-to-face assessments for incarcerated individuals. This role involves creating and maintaining service care plans, ensuring accurate and timely activity logs, and obtaining necessary medical and demographic information. The Specialist will manage a caseload of clients with opioid addiction or recent prison release, offering case management and advocacy through home, hospital, and prison visits. Key responsibilities include linking clients to medical and social services, monitoring and reassessing client needs, and collaborating with care providers on discharge and home care planning. The Specialist will also work with various agencies, attend supervisory and staff meetings, and participate in ongoing training and development.

 
Position Requirements

BSW, MSW is preferred.

Candidates should have experience in human service delivery, case management, assessment, and client advocacy, with a strong understanding of harm reduction practices and the medical and psycho-social issues related to opioid addiction. Bilingual skills (Spanish/English) and experience in corrections are advantageous. The role requires the ability to work with diverse populations and various professionals, including medical, non-medical, and para-professional staff within the case management and prison systems. Candidates must also be eligible to enter correctional facilities as a contract employee.
