Maintain accurate records to track progress.   Position Requirements Valid MD social work license (LBSW, LMSW, LCSW, LCSW-C) 2+ years experience in medical-surgical/geriatric/pediatric case management. Strong assessment and communication skills. Reliable transportation and valid driver's license.
