Community First Health Center is seeking a Physician to join our multi-location Federally Qualified Health Center in St. Clair County. In order to service our patients and community in the best way possible, we are looking for an exceptional Physician - MD/DO to join our Port Huron medical facility located at 1011 Military Street, Port Huron. Under the direction of the our Medical Director and Chief Executive Officer, you would be responsible for providing a comprehensive, community based, family centered primary care practice consisting of internal medicine, pediatrics, surgery and gynecology. Our approach is designed to include health maintenance and education as well as acute and chronic health care.

Daily responsibilities of our Physician - MD/DO may include:

• Develop a personalized care plan including medical history, physical examination, diagnosis and appropriate treatment and/or referral if necessary
• Communicate the importance of preventative medicine
• Immunize patients against communicable diseases
• Refer cases requiring specialized services while adhering to managed care contracts, principles and guidelines while using established/approved provider networks
• Maintain accurate and concise documentation of all patient encounters on a timely basis
• Enhance professional growth and development through participation in provider meetings, educational programs, current literature and professional conferences
• Other duties as assigned/requested

This is a full time opportunity; 40 hours per week. As a provider, you would be expected to see patients four (4) days per week between the hours of 830a-5p which would include a late night and rotating Saturday.

Position Requirements
  • Doctoral degree of medicine or osteopathy from an approved medical school
  • Licensed to practice medicine in Michigan
  • Family practice or internal medicine/pediatric board certification
  • DEA license
  • BLS certification
  • Excellent communication skills with the ability to establish nurturing relationships with ease
