Community Physical Therapy is a leading therapy provider in the greater Chicagoland and southern Wisconsin regions. We are seeking an experienced home health PT to provide services in the Aurora, Bloomingdale, and Elmhurst areas at Assisted and Independent Living communities. Current experience with OASIS SOCs is a must. PRN work for both weekends and weekdays are available. The HH documentation system used is Kinnser.
If you have a passion for geriatric rehabilitation and enjoy working with other dedicated therapists as part of a team to provide skilled care, this is an excellent opportunity. Your role will involve providing Home Care services to residents in their living environments. Recent Home Care experience is crucial for the success of this role. There are also opportunities to provide outpatient services in the same locations as part of CPT's Continuum of Care program.
