The Long Term Care Social Worker plays a critical role in providing comprehensive social services to residents and their families within a nursing home setting. This role involves addressing the psychosocial needs of residents, facilitating communication between residents, families, and the interdisciplinary care team, and advocating for the rights and well-being of residents.

Key Responsibilities:

  1. Psychosocial Assessment: Conduct thorough assessments of residents upon admission and on an ongoing basis to identify their psychosocial needs, preferences, and strengths.
  2. Care Planning: Collaborate with the interdisciplinary care team to develop individualized care plans that address the social, emotional, and psychological needs of residents.
  3. Counseling and Support: Provide counseling and emotional support to residents and their families to help them cope with the challenges of aging, illness, and transitions.
  4. Crisis Intervention: Respond promptly to crises and emergencies, providing support and intervention as needed to ensure the safety and well-being of residents.
  5. Discharge Planning: Coordinate discharge planning for residents transitioning to other levels of care or to community settings, ensuring a smooth transition and continuity of care.
  6. Advocacy: Advocate for the rights and preferences of residents, ensuring that their voices are heard and respected in decision-making processes.
  7. Community Resource Referral: Connect residents and their families with community resources and support services to enhance their quality of life and promote independence.
 
Position Requirements

Maryland social work licensure, experience in long term care preferred, but not required.
