Job Title: Week-End Manager

Position Reports to: Director, Clinical Operations

POSITION SUMMARY

The Week-End Manager is responsible for ensuring that patients referred for services over the weekend are scheduled and seen in accordance with the physician order. The Weed-End Manager will supervise the activities of the week-end team and rotation staff to ensure staff meet productivity targets. The Week-end Manager directly supervises the week-end administrative team to ensure efforts are coordinated and result in patient and clinician satisfaction. Assures care and services provided by the week-end team follow state and federal regulatory requirements and in compliance with standards of practice. Assures care is provided to each patient in a safe, cost-effective manner.

MINIMUM JOB REQUIREMENTS

Education & Qualifications

Education: Graduate of an accredited School of Nursing; Bachelor's degree required or at least 5 years equivalent health care supervisory experience required; Master's Degree preferred.

Licensure/Certifications: Must have current licensure by the Georgia State Board of Nursing. Must have current and valid CPR certification. Must possess a current valid Driver's license.

Previous Experience: Minimum three years of experience in health care management preferably in a home health care setting in daily operations. Requires proven interpersonal, supervisory, and management skills with the ability to communicate effectively.

Knowledge/Training: Demonstrates knowledge of Medicare/Medicaid and other funding source regulations as applied to compliance and payment for home health care services. Demonstrates knowledge of the physical, social, and emotional needs of the elderly disabled and terminally ill. Demonstrates excellent observation, verbal and written communication skills and ability to comply with clinical and financial standards and best practices. Knowledge of supervision to effectively relate with volunteers and staff. Demonstrates ability to supervise and direct professional and administrative personnel. Demonstrates ability to work with referral sources and partners and dealing tactfully with internal and external customers and public relation skills. Demonstrates knowledge of corporate business management. Organization personnel are expected to participate in appropriate continuing education as may be requested and/or required by their licensure and supervisor. In addition, organization personnel are expected to accept personal responsibility for other educational activities to enhance job related skills and abilities. All personnel must attend mandatory educational programs.

Computer Skills: Must be familiar with general use and functions of the computer, such as, usernames and password concepts; internet; e-mail; navigation of computer desktop or laptop, including starting programs, using files, and windows, effectively use navigation buttons and tool bars; ability to self-manage online HR services and online training programs.

Transportation: Must have valid automobile insurance and reliable transportation.

Other: Demonstrates understanding that to provide for safe and quality of patient care delivery, that there may be rare occasions when the BD is required to make home visits to direct, coach and/or mentor clinical staff, or in times of urgent care delivery needs of admitted patients. Maintains yearly ongoing competency requirements.

Manual Dexterity: Constantly performs simple, moderate, and difficult manipulative and gross body coordination skills such as, but not limited to, washing a patient, writing, and typing documentation, positioning a patient, inserting IV lines, taking B/P's and calibrating equipment.

Coordination: Constantly performs tasks which require hand-eye coordination such as keyboard skills, IM injections and running power tools. Frequently performs arm-hand steadiness tasks such as taking B/P's and calibrating tools and equipment.

Mobility: Constantly moves and traverses and/or remains in a stationary position to access patients. Constantly able to position self for prolonged periods in awkward or uncomfortable positions such as, but not limited to, bending over tables, reaching, twisting, climbing stairs, stooping/squatting, and/or kneeling.

Visual: Constantly able to observe, judge and estimate objects far away as in driving; closely as in reading thermometers or patient assessments; and to discriminate color as in reading microscope slides, colors in electrical wires or warning lights.

Hearing: Constantly communicates with patients, caregivers, and others; able to detect normal sounds with some background noise; and distinguish sounds as voice patterns, auscultation, and tape transcribing.

Mental Requirements:

Critical Thinking Skills: the ability to assess, analyze, and evaluate information in a clear, rationale, open-minded manner using data/metrics to draw conclusions and develop solutions

Concentrate: Constantly able to concentrate on minimal detail with little, minimal and/or constant interruptions.

Constantly able to communicate in written word, using checklists with simple sentences and using advanced written skills.

Work Environment: Work is generally performed both within an office environment, with standard office equipment available where lighting and temperature are adequate, and there are no hazardous or unpleasant conditions caused by noise, dust, etc., and within the field where these work environment characteristics are representative of what may be encountered while traveling to, inside and/or outside patients' homes and community-based group homes:

Constant safety and health hazards which may include exposure to hazardous materials such as chemicals; blood and/or bodily fluids; mold; dust and/or other sensory exposures including seasonal conditions in outside weather and temperature fluctuation inside homes.

Frequent encounter with animals thereby resulting in exposure to pet dander/saliva and potential situations which may present risk of injury.

Constant potential road/driving hazards as daily work schedule require care for multiple patients at separate locations.

Constant ergonomic hazards from patient lifting or adjustment; from carrying/pushing or pulling laptop bag and/or clinician bag; and from repetitive documentation on laptop.

JOB SPECIFIC EXPECTATIONS

Management:

Promotes, assures, and implements a philosophy of patient-centered care and services delivery.

Fosters an environment of continuous process and outcome improvement.

Develops week-end administrative and supervisory staff to independent level of functioning. Progressively counsels' staff who do not comply with VNHS policy and practices.

In conjunction with the Branch Director, provides input on the hiring of appropriately trained professional staff to ensure the delivery of safe care. Oversees the orientation of new week-end staff,

Ensures all week-end staff are competent to perform the care each patient requires.

Ensures appropriate and cost-effective care delivery by managing key metrics to benchmark: Admission conversion rate, productivity targets. Actively leads team and participates in performance improvement process with quality departments.

Conducts performance appraisals of team administrative and supervisory staff accurately and in accordance with VNHS policies and procedures.

Demonstrates effective time management skills.

Completes and disseminates all required reports. Utilizes reports to help manage visit scheduling based on patient priority.

Ensures that internal and external customer service/complaints or concerns are investigated with documentation of all aspects of the event and shared appropriately with corporate staff as required.

Coordination of Care:

Facilitates and oversees the care coordination and interdisciplinary communication activities of all team members to ensure appropriate clinical hand-offs from weekends to week-day teams.

Oversees the scheduling function including ensuring each staff has an appropriate case load, patient's needs are matched to clinician skill level, and that continuity of care for each patient meets organizational expectations

Assures all patients are seen in accordance with physician's order for care.

Assures staff availability for customer service calls and intercedes as necessary to ensure customer service expectations are met.

Communication:

Communicates effectively with peers, managers, and other staff in the home Health program and at the corporate level.

Facilitates communication between all shifts and primary staff.

Regulatory:

Ensures team members' compliance with all state and federal regulations including but not limited to Medicare and Medicaid regulations, T JC standards and utilization management.

Oversees the safe delivery of care and ensures accepted standards of care. Makes appropriate recommendations for improvement to Director, Clinical Operations and Executive Director, Home Health.

Follows all privacy policies of VNHS and maintains the confidentiality of protected healthcare information (PHI).
