Description

Provide nursing care as directed by the psychiatrist to consumers, in accordance with the standards of the Nurse Practitioner Act. This includes but is not limited to:

  • Administering medication as prescribed including coordinating medications and prescriptions and giving IM injections and Clozaril management.
  • Providing feedback to the psychiatrist and other treatment team members as needed regarding the medication aspect of treatment.
  • Writing orders for lab work with the psychiatrist's directive, and monitoring results.
  • Taking and assessing vitals signs as indicated.
  • Daily review of consumer's charts that are seen by the psychiatrist; transcribing and following doctor's orders. Which includes ensuring that the diagnosis codes are current.

Assessing for severity of symptoms, and arrange hospitalizations as needed. This includes:

  • Contacting hospitals as needed;
  • Involving MH/MR delegate or police as needed:
  • Giving instructions to consumers and/or family and/or other providers as needed.

Assist Continuing Care Staff with daily functions including:

  • Triage incoming phone calls from consumers then taking measures to resolve problems or needs
  • Transcription of Doctor's orders with follow-up
  • Receive & log in the medications delivered to the Center. Also, dispose of expired medications appropriately.
  • Complete needed paperwork for insurance authorizations.
  • Assist the Center staff in handling crises, completing forms and making community & family contacts when necessary.
  • Will complete & sign all Continuing Care Treatment Plan Updates as they are due.
  • Will obtain authorizations for psychiatrist & nursing services.

Provide coordination of care. This includes:

  • Communicating regularly with team members to facilitate consumer progress.
  • Transcribing and following doctor's orders after consumer are seen.

Stay informed of state regulations, maintain appropriate professional continuing education requirements an follow agency policies and procedures . This includes:

  • Attending weekly departmental meetings/staffings and other meetings to stay informed of current regulations, policies and procedures related to clinical and administrative practices.
  • Maintaining complete files and clinical documentation.
  • Complying with quality assurance guidelines.
  • Maintaining awareness of all Federal, State, and Managed Care Laws, policies, regulations and procedures regarding clinical care, Best Practices Guidelines, and appropriate fiscal requirements related to clinical services.
  • Attend in-services and trainings as required to maintain level of competence in nursing and mental health field.
  • Attending weekly departmental meetings/staffings and other meetings relevant to maintaining sound clinical/medical management.
  • Attending weekly supervision with supervisor and addressing all issues which impact the quality of care.

Screen & evaluate consumers. This includes:

  • Working with treatment team to provide initial & ongoing physical & psychiatric assessment.
  • Rendering nursing diagnostic & dispositional judgment to facilitate the client's movement into appropriate services.
  • Referring & obtaining more specialized testing and evaluation if indicated.
  • Provide consumers and their family's education on medication as indicated.

Conduct appropriate consultation with treatment team regarding clinical care and policies/procedures. This includes:

  • Attending regular supervision with the immediate supervisor and fully addressing all issues which impact quality of clinical care;
  • Attending and participating in weekly case reviews.
 
Position Requirements
  • Licensed Nurse
  • Act 33, 34 Clearances and FBI clearance
  • Act 31 Mandated Reporter Training
  • Good communication skills
  • Good computer skills
  • At least 2 years of professional experience working in the mental health field (Preferred)
  • One year of clinical experience (Preferred)
  • Employee must be able to understand the English language, both spoken and written.
  • Employee must be able to communicate verbally with others in a clear, understandable manner.
  • Employee must be able to communicate in writing to others in a clear, understandable manner.
  • Employee must be able to use a telephone effectively, and be able to leave and retrieve voicemail messages.
  • Employee must be able to tolerate sitting for at least one hour, and require no more than a 5-minute break for every 2 hours of work.
  • Employee must possess sufficient cognitive capacity to comprehend complex information, and be able to explain their understanding of this information in a coherent, understandable manner.
  • Employee must be able to tolerate protracted periods of psychological stress that is entirely expected when providing assessment/treatment to consumers who may be violent, demanding, and suicidal or have severe psychiatric disorders.
  • Employee must be able, under unusual circumstances, to work outside of their regular hours, take calls at home in emergencies, and perform crisis intervention when special demands require it.
  • Position requires that employee be able to conceptualize and discuss consumer cases in a small group setting.  Therefore, employee must be able to understand psychotherapeutic concepts and relate them effectively to their cases, think abstractly, and effectively express him/herself verbally. 
  • Position requires that employee be able to sensitively communicate confidential information in appropriate ways to family members and service providers both inside and outside Community Guidance Center.  Therefore, employee must be able to understand and abide by agency policy and procedure, and effectively ascertain what is appropriate to disclose in a variety of situations and settings.
