Position Summary: The Intake Coordinator is the main point of contact for participants seeking to enter the Safe Homes Strong Communities rapid rehousing program. This position is responsible for answering inquiries, assessing eligibility, and guiding new participants on resources, options and next steps, helping with a smooth transition to permanent Safe Homes Coordinator. The Intake Coordinator also carries a small caseload of longer-term participants enrolled in third party rental assistance and voucher programs, providing safety planning, service coordination, advocacy and support. Participants in the Safe Homes Strong Communities program are all victims of Intimate Partner Violence and have experienced complex traumatic events that shape how they interact with the people around them. The Service Coordinator must have a firm understanding of trauma, the effects of trauma, engaging traumatized people, and must be comfortable working in a Voluntary Services program model.

Standard Schedule: Standard hours for this position are Monday-Friday, starting between 9:00 am to 5:00 pm. This position is based on-site in HRM's Administration Building location in Baltimore City, Maryland. This role includes up to 20% travel around the Baltimore Metropolitan area.

Primary Responsibilities:

  • Triage all pending participants' program inquiries and make introductory contact within 24 hours of receiving referrals, provide a thorough assessment of needs, and coordinate with internal and external providers of services.
  • Complete the secondary screening process for potential participants to enter the Safe Homes waiting list, sharing additional services and resources and diverting persons to emergency and community resources as needed.
  • Field program inquiries from staff, community partners and persons seeking to join the program. Educate staff and community about eligibility, timeline, resources and process.
  • Transition participants to permanent Service Coordinators or to alternative programs when ready.
  • Maintain a caseload of up to 20 participants, offering service coordination for those receiving third party rental assistance and voucher programs, conducting needs assessment, providing safety planning, developing culturally sensitive service plans with timelines, and collaborate with service providers internally and externally. Connect exiting participants with other agency services and outside local resources.
  • Visit participants in home environments determined to be safe, including temporary hotel placement.
  • Provide one on one counseling for participants in crisis and to help participants resolve conflict.
  • Distribute material goods according to program procedure like clothing, bus tokens, and personal hygiene supplies.
  • Guide and support participants to increase self-sufficiency skills, safety, education on domestic violence, and problem solving.
  • Maintain paper and electronic case files in keeping with program standards.
  • Participate in Safe Homes Team meetings and make recommendations concerning individual participant's length of stay in the program and eligibility for financial assistance.
  • Provide relief and back-up coverage in emergency and urgent situations if required.

Authority:

  • Distribute material goods to program participants.
  • Determine eligibility and enrollment in Safe Homes program.
  • Create service plans with participants. Review and adjust plans as relevant.
  • Make offsite visits, and visits to agency providers and landlords.
  • Distribute participant material and financial stipends in accordance with budget and individual participant plan that has been approved by program management.
  • Recommend participants for admission to HRM programs, including Emergency Shelter or 3 day emergency hotel placements pending space.
  • Represent the agency with area service providers and landlords.
  • Arrange for interpreters for participants as needed.

Accountability/ Measures of Success:

  • Meet program quality and service targets; including caseload performance standards, and deadlines for data entry efforts and reports.
  • Phone calls and email inquiries are responded to within 24 hours.
  • Individual Service Plans are completed within 24 hours and program assessments within 48 hours of program referral.
  • Participants meet as scheduled and are well educated on all available services.
  • Participants feel welcomed and receive services that are culturally appropriate and sensitive.
  • Material goods and financial assistance are disbursed to participants as outlined and have appropriate documentation to justify expenditures.
  • Follow up visits are conducted in accordance to program standards and documented accordingly.
  • Participant's cases are documented clearly, concisely, and in keeping with agency accepted professional standards.

Experience/ Requirements:

  • Two years of paid or volunteer experience providing counseling, case management, or victim centered advocacy. Associate degree in Human Services field may substitute for one year experience.
  • Excellent oral and written interpersonal skills with a strong ability to relate to a diverse range of individuals including victims, children, donors, business leaders, human service providers, and donors.
  • Excellent time management and organizational skills.
  • Clean driving record, current state driver's license, and reliable personal transportation.
  • Must pass Criminal Background Check.

Highly Preferred Skills:

  • Oral and written fluency in more than one language (Spanish, French, Urdu, or Arabic highly preferred).
  • Experience in direct service provision with the Latino immigrant community.
  • Knowledge and understanding of intimate partner and/or sexual violence.
  • Knowledge of local human services programs/ agencies and housing resources.
