We believe vaccination is the best protection for our patients, community and Visiting Nurse staff.   Location Community Care   Category Community Care   Description The Lead Care Manager, SOURCE supports the on-going operation of the SOURCE program by providing clinical consultation to staff through preceptor training to new staff; one-on-one assistance to peers and on-going enhanced care management activities necessary to meet the needs of assigned Members. This position is responsible for providing support to other Care Managers as it relates to assistance in completing home visits (quarterlies) and monthly telephone contacts when Care Managers are on leave. This position will provide Care Managers relief, when necessary, as it relate to coordinating services and monitoring services in Primary Care Manager’s absence. Clinical outcomes will be evaluated to ensure that services are provided in a professional, comprehensive, and cost-effective manner. The person holding this position will maintain a caseload up to 50 members.   Full-Time/Part-Time Full-Time   Position Requirements Education & Qualifications: Education: Bachelor’s Degree in Social Work, Sociology, Psychology, or a related field required Graduate of an accredited Diploma, Associate Degree, or Baccalaureate for Social Worker. Preference is given to individuals with master’s degree. Licensure/Certifications: Current CPR certification required Previous Experience: minimum 2 years’ experience in case management in a health care field. Preference given to national certification in case management. Knowledge/Training: Knowledge of Medicare, Medicaid and other funding source regulations as applied to service program. Demonstrates knowledge and competence in advanced practice nursing skills, including the nursing process, and collaborating with interdisciplinary team Skills: Must be able to function independently and efficiently in community-based environment. Requires proven interpersonal skills with an ability to communicate effectively. Demonstrates proven decision skills, organizational, self-discipline, and time management skills. Must be skilled in physical assessment and development of care plans. Computer Skills: Must be familiar with general use and functions of the computer, such as, usernames and password concepts; internet; e-mail; navigation of computer desktop or laptop, including starting programs, using files, and windows, effectively use navigation buttons and tool bars; ability to self-manage online HR services and online training programs.
