SUMMARY

Provide nursing services that contribute to the health and well-being of each individual. The Licensed Practical Nurse (LPN) will participate in the assessment, evaluation and follow through of recommendations made by the Person Centered Plan team and physician. Facilitate proper nursing care, review of medication administration and promoting quality of care for participants. Participate in the development of individual health care protocols as part of the PCP process. Provide nursing care per state and Nurse Practice Act requirements.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  1. Provides services to all individuals assigned as per plan. Seeks to provide assistance to other individuals and team members when needed. Participates in PRIDE room services as part of the team.
  2. Assists all individuals towards the achievement of their goals. Assists all individuals to perform and participate in scheduled activities and in Activities of Daily Living (ADL) as needed.
  3. Completes required documentation that supports care and services provided including but not limited to ID notes.
  4. Ensure equipment is clean and functioning.
  5. Maintains a clean and safe work environment.
  6. Anticipates safety needs for all individuals and team members.
  7. Complies with all Federal, State, local regulations and requirements, and company policies and procedures.
  8. Carry out habilitation assistance services as required by the participant’s Person Centered Plan (PCP).
  9. Maintain appropriate documentation
  10. Follow all medication administration procedures; documentation to be completed efficiently.
  11. Assist the Manager and Director with special projects.
  12. Supports and participates in department activities.
  13. Establishes cooperative relationships with team members to determine effective strategies to achieve individual and department goals.
  14. Assist the Nursing Services Manager in Developing appropriate Care Plans for each participant.
  15. Assist with intake and screening prospective participants.
  16. Assist with initial Nursing Assessments with the RN.
  17. Assist with establishing chart for new participants.
  18. Administer medications and treatments per prescription orders, document on medication sheets and report adverse reactions.
  19. Receive medications from family as needed, ensuring that all medications are logged and secured.
  20. Provide proper disposal of discontinued or expired medications and maintain destruction log.
  21. Maintain narcotic sign out sheet and count for controlled substances.
  22. Observe participants for condition change. Note in participants charts and report book, communicate changes to family/caregivers or Physicians, if needed.
  23. Assess and triage in an emergency, treat when possible; call 911 when necessary.
  24. Assist with serving meals and feeding participants as needed. Monitor meals ensuring that special diets are adhered to.
  25. Assist with loading and unloading buses as needed.
  26. Teach proper techniques and sanitation as needed.
  27. Utilize appropriate methods for working with individuals with intellectual and or physical disabilities.
  28. Report suspected Abuse, Neglect, Exploitation, Isolation, and Abandonment (ANEIA) immediately as mandated by state law.
  29. May be assigned other duties as needed.

 

SUPERVISORY RESPONSIBILITIES

This position does not directly supervise others.

 
Position Requirements

SKILLS REQUIRED

  • Ability to communicate effectively with individuals, team members, and supervisors (verbal and written).
  • Ability to read, analyze and interpret written instructions and contract specifications. 
  • Ability to respond to common inquiries and complaints from individual workers with disabilities, contractors, representatives and other team members. 
  • Ability to effectively present information to fellow team members and others.
  • Must possess social perceptiveness to assess and understand other’s reactions and behaviors.
  • Proficient computer skills including: Microsoft Word, Outlook, Excel.
  • Ability to fill out all required documentation including but not limited to: ID Notes, Incident Reports, etc.
  • Ability to define problems, collects data, establish facts and draw valid conclusions. 
  • Ability to interpret instructions and deal with multiple variables.
  • Ability to resolve multiple complex issues simultaneously while remaining calm in stressful situations.

 

EDUCATION AND EXPERIENCE

  • Required: High School Diploma or equivalent.
  • Required: Current LPN license.
  • Required: Minimum of two (2) years of experience working with people with disabilities.
  • Required: Current CPI certification within 30 days of employment
  • Required: Current CPR and First Aid Certification within 30 days of employment
  • Preferred: Experience working with individuals with disabilities.
