Valley Community Services Board serves as the single point of entry into publicly funded mental health, developmental, and substance use disorder services for these localities.

Description

The individual in this role is responsible for providing outreach services to homeless mentally ill persons within the VCSB catchment area. The PATH staff shall engage with homeless persons wherever they may be found within the area, develop a working relationship with them and assist them with accessing housing and other critical needs. She/he is expected to perform job duties with a substantial degree of independency, exercise sound judgement, maintain professional conduct at all times, and develop positive alliances with consumers. This position requires a high degree of initiative, independence, problem solving skills, and strong verbal, written, and computer skills.

Essential Functions

  • Provide outreach services to persons experiencing homelessness and serious mental illness or co-occurring disorders,  wherever they are located, engage with them, and link to various community housing, medical, social service, and treatment services.
  • Participate in presentations to various community agencies that may provide assistance to homeless persons to educate them about this resource and to solicit referrals and coordinate services.
  • Assist consumers with referrals and applications for financial, housing, medical, and other benefits.
  • Document the provision of services to consumers on a daily basis, develop individualized service plans, and quarterly reviews.

Minimum Qualifications

All applicants must meet the following minimum qualifications in order to be considered qualified for the position:

  • Bachelor's Degree required.
  • Must have the knowledge, skills and abilities as outlined for the position.
  • Qualified Mental Health Professional - Adult (QMHP-A) preferred.
