The social worker, working alongside the Director, will oversee the Social Services Department, conducting resident and family interviews, involving them in program planning, and addressing social and environmental issues. They will also assist in resolving problems such as seeking financial aid, discharge planning (in collaboration with community agencies), and making referrals to specialized community services as needed. Additionally, they will coordinate with various departments, participate in Facility Surveys, and communicate residents' needs to the medical team.   Position Requirements NJ CSW or LSW with at least 1 year of experience working in a Skilled Nursing Facility.   This position is currently accepting applications.
