We believe vaccination is the best protection for our patients, community and Visiting Nurse staff.   Location Community Care   Category Community Care   Description The HCBS Program Manager supervises care coordination personnel and acts as the liaison between care coordination and the Executive Director of Community Care. This individual will ensure that the performance of team members is evaluated on an ongoing basis through periodic case file reviews as needed, and coordinate training for new employees so that they accompany the team lead and more experienced staff within the first 6 months of employment. They will submit a monthly schedule of daily activities for the first month of employment for new SW team members to the Clinical Director.   Full-Time/Part-Time Full-Time   Position Requirements Education: Bachelor’s Degree in Social Work, Sociology, Psychology, or a related field; or a graduate of an accredited baccalaureate school of nursing. Master’s degree preferred. Licensure/Certifications: SW licensure preferred, but not required. Must have current CPR certification. Previous Experience: Two years experience in care management activities, preferably in the program for which supervision position is sought. Supervisory experience preferred. Knowledge & Training: Must have demonstrated knowledge of program policy and procedure. Requires proven interpersonal skills with ability to communicate effectively and negotiate problems and/or complaints. Demonstrates proven decision-making, organizational, and management skills. COMPUTER SKILLS: Must be familiar with general use and functions of the computer, such as, usernames and password concepts; internet; e-mail; navigation of computer desktop or laptop, including starting programs, using files, and windows, effectively use navigation buttons and tool bars; ability to self-manage online HR services and online training programs.
