Join our team as a Case Manager and make a meaningful impact on the lives of individuals facing incarceration, opioid addiction, and re-entry challenges in the Philadelphia County Prison system. In this role, you will provide comprehensive case management services, offering support to incarcerated individuals, those battling opioid addiction, and individuals recently released from prison. Your responsibilities will include consulting with staff on prison-related matters as needed and serving as a liaison with the Defenders Association, court officers, and probation/parole offices.   Position Requirements BSW or MSW with Experience in human service delivery, case management, assessment, and client advocacy. Understanding of medical and psycho-social issues related to opioid addiction and other health concerns. Preferred experience working with incarcerated and/or re-entering populations. Ability to work effectively with diverse populations. Working knowledge of local resources. Flexibility in work hours, including occasional evenings and weekends.
