JOB SUMMARY: Two primary functions:

Serving as the Navigator of the Hancock Health Congregational Network (HHCN), facilitating collaboration and partnerships between congregations, the community, and healthcare providers.

  • Establishing and sustaining relationships and partnerships with faith communities within Hancock County, enrolling them in the HHCN.
  • Serving as the main point of contact for Liaisons within all churches in the HHCN and provides ongoing training and support.
  • Collaborates with the Liaisons to discover health needs and identify resources, programs and activities that can be provided to HHCN members.
  • Upon patient admission, assesses HHCN members to determine additional needs that their faith community can assist with. With patient permission, the Navigator contacts the appropriate church Liaison to connect the patient to additional support. The Navigator facilitates communication among clinical and transitional care coordinators, the patient, and the congregational Liaison to ensure a smooth transition to recovery at home.
  • Actively promotes a holistic approach to health care and works to ensure all involved in the Network have resources and supports necessary to live a healthy life.

Providing chaplaincy services as determined by the Director of Chaplaincy Services. These services include but are not limited to:

  • Day-to-day spiritual and emotional support to patients, associates, and physicians.
  • Participation on multi-disciplinary health care teams for the delivery of holistic care (body, mind, and spirit).
  • Screening patients for spiritual distress and conducting necessary spiritual histories and assessments with specific application to the overall patient care plan.
  • On-call chaplaincy services outside of regular business hours (5p-8a weekdays/ 24-hours weekends) and during times when the Director of Chaplaincy Services is away from the hospital campus.
  • Documenting the delivery of spiritual care in the medical record.

  • Proficiency with computers, including Microsoft programs (Word, Excel, Publisher, Power Point, and Outlook).

CHAPLAINCY

  • Assess for emotional and spiritual distress and provide appropriate support

  • Administers religious services and ceremonies when requested or arranges for clergy to do so.
  • Listens actively to family members and offers supportive counsel, especially in times of crisis or impending death.
  • Conducts spiritual assessments when requested by patients or staff.
  • Responds to all codes in hospital.
  • Charts appropriately, effectively, and in a timely manner on all patients visited.
  • Functions as an ethical and religious resource to hospital staff.
  • Provides individual support and spiritual counsel to associates as requested by them and/or their supervisors.
  • Participates in departmental meetings. Serves as a member of committees and/or task forces as assigned.
  • Works on special projects as assigned by Director of Chaplaincy Services.
  • Participates in appropriate support groups offered by department to the community.
  • Adheres to the Chaplain’s Code of Ethics and HRH policy on confidentiality and discrimination.
  • Adheres to Hospital Regulations and follows Chaplaincy Services policies and procedures with a positive attitude.
 
Position Requirements

LICENSURE/CREDENTIAL REQUIREMENTS:

  • Completion of the educational requirements expected within one’s religious community for pastoral leadership.
  • Ecclesiastical Endorsement by one’s faith group as a Health Care Chaplain.
  • General education should be a minimum of high school and a four-year undergraduate degree.
  • A minimum of two (2) years pastoral work experience in a congregation or other setting is required.
  • Board Certification, Credentialing, or specialized training in Chaplaincy/Pastoral Care leading towards certification/credentialing with an officially recognized professional chaplaincy organization.
