Essential Functions Provides intensive case management, rehabilitation, and supportive services to individuals experiencing serious mental illness. Work with a population of adults who may be dually diagnosed with chronic mental illness and substance use issue. Services are provided in accordance with the ACT model of treatment. Services are provided in multiple settings, including in the home and in the community. Requires working a schedule rotation of evenings, weekends, and holidays. Limited on-call responsibilities. Minimum Qualifications Bachelor or Master's Degree in human services field and one year direct clinical experience with mentally ill population. Valid VA driver's license with an acceptable driving record.  Must be QMHP or QMHP-T eligible.  Physically able to climb stairs and assist with moving small to moderate sized items. Able to carry out job responsibilities within the existing setup of the ACT office.
