The MRST Team Leader is responsible for the day to day operation of two
MRST Teams serving central and eastern Suffolk County. The primary goal of the service is to provide solution focused brief intervention to individuals with high needs to maintain themselves successfully in the community; and to assist individuals by providing enhanced services to decrease their need for emergency services and hospitalizations.
Duties and Responsibilities:
Supervision of multi-disciplinary team of staff in the provision of supportive services to individuals with mental health disabilities residing in OMH sponsored Supported Housing programs in the east and central Suffolk County catchment area.
Provision of weekly individual and group supervisory meetings with staff, including weekly client and documentation review. Written record of supervisory meetings will be maintained, and are subject to review by Concern administrative staff as well as any funders and stakeholders.
Outreach to community and other service providers to ensure that all potential referral sources are aware of the services available in an effort to stimulate referrals to maintain full census.
Provision of direct supportive services (service planning, medication monitoring, assistance in forming linkages and/or referrals to clinical services, day programs, vocational services and opportunities, community integration, entitlements, etc.) to a reduced caseload of clients enrolled in the service.
Responsible for maintaining oversight of all client files and other relevant documentation in compliance with agency standards.
Providing required monthly reports to OMH and Suffolk County.
Ensuring that services are being provided in compliance with OMH and Suffolk County requirements as per grant instructions.
Crisis intervention.
Remain current with all Agency required compliance documents and trainings.
Other duties as assigned by the director of the program.
Positions Supervised: Wellness Specialists, Substance Abuse Specialists, Licensed Practical Nurses, Peer Counselors and ADL counselors.
Salary: $65,000/yr.
