Group Home Social Worker needed for full or part-time, direct hire or temp to perm hybrid position in Burtonsville, Annapolis and Harwood, MD.
The Social Worker is responsible for providing social work services to clients and their families in the Group Homes; for coordinating treatment with the Program Manager and staff; for coordinating treatment with the Placing Agent and outside service providers. A Social Worker with two years post-Masters experience may supervise a BSW Intern; a Social Worker with a LCSW-C may supervise an MSW intern. The Social Worker’s specific activities may vary from facility to facility.
ESSENTIAL DUTIES AND RESPONSIBILITIES:

Deliver weekly individual and group therapy to residents and/or monitor therapy provided by community providers;

Provide on-site crisis management and de-escalation;

Treatment planning with the facility staff and Placing Agency staff to include:

Developing and updating the individualized Treatment Plan

Facilitating Treatment Team meetings

Provide written and/or verbal assessments and updates of residents.

Maintain phone/video/electronic/in-person contact with families/care takers/Treatment Team members of residents.

Monitor resident case files and participate in case file audits.

Complete thorough and timely clinical documentation.

Participate in the Referral process to include reviewing referral packets and interviewing potential residents.

Engage in required agency and state trainings.

Supervise Social Work Interns, if applicable.

Participate in clinical staff meetings.

Participate in weekly clinical supervision.
Salary/Compensation - $30-$40/hr. $62,400-$83,200/year
