Description

Valley Community Services Board is looking for a DAP Mental Health Case Manager. The MHCM staff provides ongoing community-based mental health case management services to the long-term mentally disabled population within the Staunton/Waynesboro, Augusta, and Highland County catchment areas.This staff will work with individuals participating in MHCM services with an active Discharge Assistances Plans (DAP). Services include but are not limited to: referral to other agencies, supportive counseling, outreach, crisis intervention, and assessment.
He/she is responsible to and supervised by the Adult Mental Health Case Management Supervisor. He/She is responsible for carrying out client treatment plans developed from initial and ongoing assessment and clinical staffings. He/she is responsible for documenting to Medicaid and licensure standards and adhering to professional standards of ethical treatment. Confidentiality will be adhered to in accordance with VCSB policies and legislation.

Essential Functions

  • Provides referrals to other agencies, supportive counseling, outreach, crisis intervention, and assessment.
  • Carries out client treatment plans developed from initial & ongoing assessments and clinical staffing.
  • Assists clients in obtaining and maintaining disability and social entitlements and any associated benefits.
  • Monitors and coordinates with DAP liaison to maintain DAP benefits.
  • Monitors and coordinates with VCSB Rep Payee to manage the consumers budget and personal needs as appropriate.
  • Advocates and promotes recovery-oriented practices.
  • Ensures Service Plans reflect client strengths, need and interest.
  • Ensures service goals promote client self-determination and independence.
  • Utilizes office based and or community visits to provide support counseling and case management services.
  • Initiates and maintains collateral contacts with other treatment providers within in VCSB as well as outside the agency.
  • Provides consultation and coordination of care with family and other providers.
  • Provides outreach to clients via home visits, telephone calls or meetings in the community.
  • Assists VCSB crisis team in providing crisis intervention services when clinically appropriate.
  • MH Case Management Services for clients with DAP funding
    • Utilizes office-based and/or community visits to provide supportive counseling, and case management services including linking, referring, and problem-solving to clients with DAP funding at least monthly. Face to face contacts must be completed at a minimum of every 90 days.
    • MHCM assists DAP clients with finding appropriate placements if client needs higher level of care or is presented an eviction notice from placement or residence.
    • Provides consultation and coordination of care with other treatment providers with in CSBs and private providers, also to the family and significant others of clients as appropriate.

Minimum Requirements

  • Bachelor's degree in human services or related field.
  • Previous work or internship experience that provides the necessary knowledge, skills, and abilities.
  • Must pass required pre-employment screenings, including, but not limited to, criminal history and experience verification.
  • Current Virginia driver’s license with zero points or more.

Preferred Qualifications

  • Qualified Mental Health Professional (QMHP) or Licensed Mental Health Professional (LMHP) certification preferred but not required.
  • Previous experience serving adult clients with mental illness.
