We are looking for a self-driven compassionate Chaplain to join our Hospice Department. We offer a competitive salary and benefit package. Shift: PRN We are looking for a PRN Hospice Chaplain to assist with emotional and spiritual support of hospice patients and their families. Compassion, caring, and integrity is a must in this role. Responsibilities include: Conducting a spiritual assessment within 5 days of admission to the hospice program Assisting in developing and implementing interdisciplinary spiritual aspects of patient care plan. Provide spiritual support focusing on the patient/family religious preferences or spirituality. Collaborate with local clergy for the patient. Maintain and submit written clinical records in compliance with agency policies and regulations. Including the evaluation, the plan of care, and any communication notes. Participate in IDG, lead religious services of prayer, worship and rituals for patients/families as appropriate. Provide funeral or memorial services for patients as requested, and assume responsibility for future self-development.
