Every employee of Stockbridge Munsee Community is expected to present themselves in a professional manner to customers as well as other departments. Stockbridge Munsee Community strives to provide a positive team environment where everyone contributes.

GENERAL RESPONSIBILITIES: The Elder Social Worker will support elders and their families to ensure that Tribal Elders are safeguarded from harm and to improve outcomes in their lives. The position will act as an advocate to ensure the welfare of the elderly, improve the quality of life, and help alleviate some of the negative aspects of aging. Services are provided based on an individual needs’ assessment and service plans. The Elder Social Work will explain and provide information to services, assist with application processes. The Position will develop a case management plan for each client. The Elder Social worker is expected to work on a highly independent basis. The Elder Social Worker is to network with internal and external resources to provide access to services on behalf of the Elders.

STANDARD QUALIFICATIONS:

All employees of Stockbridge Munsee Community must meet the following qualification:

  1. Must submit to and pass a pre-employment drug and health screening.
  2. Must be at least 18 years of age, unless otherwise stated on job description 3. Must maintain an acceptable departmental attendance record
  1. Must be able to work weekends, nights and holidays.
  2. Must obtain and maintain Elder/Youth License as stated on job description.

 

DUTIES:

 

  1. Shall execute duties while maintaining the highest level of confidentiality.
  2. Receive and investigate instances of elder abuse and / or neglect.
  3. Shall work with a high degree of independence in administering services and in using tribal or community resources.
  4. Shall develop appropriate service plans for client(s), and families as needed in elderly services.
  5. Shall provide support services for program elders.
  6. Shall assist clients with the utilization of community resources; interpret rules and regulations and policies for the clients pertaining to resources being sought.
  7. Shall make necessary home visits to carry out case management assignments, assisting with transportation, referring to counseling, medical and dental care, hygiene assistance and all needs that may arise with an elder.
  8. Shall maintain case records and documentation.
  9. Shall provide the appropriate departments with reports as needed, and necessary.
  10. Shall assist with community outreach to the client population as needed.
  11. Shall attend in-services and other training courses designated to further understanding family and elderly case management process and or case management.
  12. Shall assist in the development of standard operations procedures and policies for the Elderly Social Worker operations.
  13. Shall provide direct services for applicable clients and other related services in homes, clinics, job sites, and other community locations within theStockbridge Munsee community, and surrounding areas as necessary for clientele.
  14. The above duties and responsibilities are not an all-inclusive list but rather a general representation of the duties and responsibilities associated with this position. The duties and responsibilities will be subject to change based on organizational needs and/or deemed necessary by the supervisor.
  15. Shall be available for local and out of the area travel as required for job related training
 
Position Requirements

QUALIFICATIONS:

 

  1. Must have a Bachelor’s degree with a major in Social Work from an accredited University(certified or eligible for certification as a Social Worker in Wisconsin, pursuant to Wisconsin Stats 457.08). A Major in Social Work with completed certification is preferred.
  2. Must have at least one year of direct job experience or training working with elders performing case management.
  3. Have knowledge, experience and understanding about elder social work, Adult Protective Services, Adult Court guardianship, Assessing vulnerable adult and families around the issues of family dysfunction.
  4. Have an understanding on how to address personal, social, and environmental challenges that develop as people age and help elderly clients find solutions to these issues. These challenges include physical complications, mental health and cultural challenges.
  5. Demonstrates the ability to work effectively with Native American people in culturally diverse environments.
  6. Exhibits the ability to manage time well and work under stressful conditions with an even temperament.
  7. Displays the ability to establish and maintain harmonious working relationships with other employees and the public.
  8. Demonstrates the ability to understand and follow oral and written instructions.
  9. Displays the skill and depth of job knowledge in assessing problem situations and formulating plans for service.
  10. Must adhere to the Tribe’s Drug and Alcohol-Free Workplace Policy during the course of employment.
  11. Must be eligible for coverage under the employer’s liability insurance.
  12. Must have demonstrated ability to maintain satisfactory working record in any prior or current employment.
  13. Must be able to meet physical requirements of position.
  14. prior to and during the course of employment with the Stockbridge-Munsee Community.
  15. Must have a valid driver’s license, reliable transportation, and insurance are required. Must obtain a Wisconsin driver’s license within 30 days of employment if applicant has an out-of-state driver’s license. Must meet and maintain the eligibility to operate a personal or tribal vehicle under the driver acceptability guidelines as established by Mohican Nation Insurance.
  16. Must abide by departmental and organizational safety, testing, and uniform guidelines.
