Responsibilities include planning for pre-admission and discharge, conducting psycho-social assessments, completing relevant sections of the MDS, and taking a holistic approach to well-being. The role involves participation in resident care planning as part of an interdisciplinary team, counseling residents and their families, coordinating community resources, and advocating for resident rights.   Position Requirements NJ CSW or LSW with knowledge of MDS assessments, admissions, care plans, and discharge processes.
