What You Will Be Doing
We want people to join our team who share our values of love, excellence, respect, empowerment, and well-being. Primary job responsibilities involve the gathering, monitoring, and reporting of outcomes data and activities related to the completion of the required activities of the OhioRISE CME. Health Data Specialists may work remotely or from an office location but will be expected to engage with care coordination and management team members via telephone, video conferencing, and electronic communication. While some meetings may be primarily face-to-face, remote team members may be able to attend virtually. Health Data Specialist will monitor a large population of individuals and families/ caregivers being supported by the organization's care coordination and care management entity activities. Health Data Specialist will be responsible for ensuring all required activities and reporting is complete for all individuals in their population pool and work with the assigned care coordinator to address any gaps. Health Data Specialists may interact with staff from other provider organizations or community organizations to obtain or exchange necessary records or documentation, but their primary communications will be with the care coordination teams.
Required Education/Experience:
- High School or GED required.
- One to three years of experience working with health data, data analytics, health outcomes reporting, or data reporting tools.
Skills, Knowledge & Abilities
- Excellent communication skills, both oral and written.
- Excellent organizational skills with the ability to stay focused and meet deadlines.
- Ability to prioritize multiple tasks.
- Intermediate computer skills with the ability to learn new software and reporting systems.
- Ability to follow all company policies and procedures.

- Professional telephone etiquette.
- Must be able to show tact and diplomacy when dealing with parents, clients, peers, and staff.
