GENERAL DESCRIPTION OF POSITION
Provides physical therapy services to clients of Pathfinder Schools, Inc. according to a written medical physicians' prescription/referral, and according to an Individualized Program Plan (IPP) or an Individualized Family Service Plan (IFSP). Supervises, trains and evaluates any physical therapist assistant assigned.
FLEXIBLE (APPROX 3 DAYS PER WEEK)
ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Provides individual physical therapy to clients.
2. Performs evaluations to determine need for services in the area of physical therapy.
3. Develops, implements, and modifies treatment plans as needed.
4. Records accurate client treatment/billing information.
5. Attends initial, annual, and semi-annual conferences with client's families as schedules.
6. Participates in Team meetings with educational and professional staff.
7. Consults with and provides training as necessary to medical and educational professionals, paraprofessionals, and parents regarding client care.
8. Informs Therapy Services Coordinator of supply, materials, and equipment needs.
9. Serves as clinical instructor to physical therapy students as assigned.
10. Supervises physical therapy assistants as needed.
11. Attends continuing education workshops with consideration for needs of the agency.
12. Determines adaptive equipment and orthotic/prosthetic needs of clients and schedules assessments as needed.
13. Perform any other related duties as required or assigned.
