Description

JOB SUMMARY:

Coordinates pre-intakes, registration process, and case management linkage to appropriate community services for the behavioral health programs of Catholic Charities, Diocese of Trenton.

DUTIES AND RESPONSIBILITIES:

  • Coordinates pre-intakes, registration process, and case management linkage to appropriate services for the behavioral health programs of Catholic Charities, Diocese of Trenton.

  •  

    Receives and screens initial requests for services offering guidance with crisis navigation to internal and external parties as appropriate.

    Assuring department coverage Monday - Friday  11am to 8pm 

    Provides case management linkage for consumers to appropriate Catholic Charities services.

    Reviews psycho-social materials to determine appropriate level of care

    Knowledge of various insurances and ability to adequately explain benefit information 

    Provide detailed description of agency programs

    Coordinate follow-up with referral sources 

    Other duties as required 

     

 
Position Requirements

REQUIREMENTS:

QUALIFICATIONS

EDUCATION and/or EXPERIENCE 

  • Bachelor’s degree in human behavioral science such as social work, psychology, sociology, and other related fields.
  • Related work experience preferred.
  • Spanish Speaking preferred, not required
 
Exempt/Non-Exempt Non-Exempt  
Weekly Work Hours 40  
Proposed Salary up to $45,000/annually  
Schedule Monday- Friday 11:00am- 8:00pm
