Case Manager - Homeless Outreach

Wyandot BHN is seeking a dedciated the Case Manager will provide goal directed intensive services to adults with co-occurring severe mental illness (SMI) and substance use disorders who are experiencing chronic homelessness, for the purpose of meeting basic human needs, increasing functional skills, decreasing the need for hospitalization thorough helping to manage mental health symptoms and maximizing independent functioning and housing stability in the community. Services will be provided in the consumer’s home, in the community, and in places where people experiencing homelessness are located.

Accountabilities:

  • Frequent locations where individuals experiencing homelessness can be found, such as shelters, day shelters, food kitchens, Frank Williams Housing Resource Center, and other homeless service provider agencies.
  • Establish a base of trust and rapport with each client, convey hope and respect.
  • Support clients experiencing homelessness in gaining safe, affordable housing.
  • Assist clients with learning and practicing daily living and social skills.
  • Assist clients in completing daily living tasks as needed.
  • Assist clients with problem-solving daily living situations.
  • Suggest other agency resources that might benefit the clients (peer support, vocational services, outpatient services) in coordination with others on the team for referral and connection.
  • Use motivational interviewing techniques to actively engage clients in services.
  • Identify strengths and needs of clients and work to involve them in recovery oriented planning for desired services.
  • Work to fully integrate clients into the community.
  • Identify and address barriers to recovery.
  • Work closely with families of clients to provide education and support.
  • Provide transportation and accompany clients to and from activities and/or appointments to provide medically necessary direct care and support, as directed.
  • Complete all required documentation thoroughly, on time and according to agency standards.
  • Provides crisis prevention and intervention as needed.

Qualifications:

  • Bachelor’s degree with emphasis preferably in social work, psychology, rehabilitation or related field, or four years of equivalent experience in community mental health setting.
  • Familiarity with mental health theory, emerging promising practices, and Evidence-Based Practice.
  • Knowledge of trends and issues, laws and regulations related to the development and delivery of case management services helpful.
  • Ability to establish relationships with individuals and organizations of influence within the social services community, including civic groups, charitable agencies, related government entities, fundraising sources, etc.
  • Demonstrated interpersonal and communication skills.
  • Emotional capacity to control and manage self if clients act out or behave in a verbal or physical manner.

Pay Range Salary Range: 52,000k-54,000k Yearly
