The position is responsible for planning and administering the social service programs within the facility in conjunction with the Director of Social Services. Assist in developing facility policies regarding participation in community planning for health and welfare services. Plan and assist in research projects and coordinate or assist with the discharge planning and coordination of community resources.   Position Requirements NJ CSW – Knowledge of discharge planning and MSW assessments.
