Immediate need for 2-3 months. M-F, 9-5:30 pm. Social worker would be responsible for the coordination of services for a multidisciplinary team approach to patient care in post-acute inpatient rehabilitation, such as stroke, brain injury, etc.   Position Requirements Requires LSW or LCSW.
