Description

The Family Support Resource Worker (FSRW) works with local Food Banks to provide vulnerable families with children (between 0-18 years old) with wrap-around family support. The FSRW provides service navigation and coordination, parenting support and education, connects families and facilitates their access to early years services, family supports and other social services.  

Carlington Community Health Centre serves children and families facing multiple challenges, addressing specific social determinants of health in a manner that is culturally sensitive, respectful and holistic in approach, grounded in attachment principles and a trauma-informed practices.

Program-Related Activities

  • Works with local Food Banks to connect with vulnerable families that would benefit from wrap-around family support.
    • Creation and facilitate quarterly meetings with local foodbank partners to optimize program delivery, client service,  share ongoing outcomes, identify and work on solutions for any gaps or issues

    • Create tools to track client engagement/outcomes, program outcomes, partner satisfaction.

    • Provide individual client needs assessments, action plans, one on one support.

  • Completes needs assessment and jointly identified need priorities.
  • Manages caseload (integrate/coordinate resources, crisis intervention, supportive counselling, information and referrals, supports parents, and provides advice on parenting).
  • Provides one-on-one family support focused on parenting and based on jointly identified family needs.
  • Plans and delivers programmes that enhance child and family welfare, such as cooking programs, parent education in a group format and/or individually, family visits, drop-ins, and others as needs are identified.
  • Actively connects families to resources and services in the community, as appropriate.
  • Identifies high need families in service area and link to family support and related services, as appropriate.
  • Develops and maintains up-to-date resource materials relevant to families with children.

As a condition of employment, the successful candidate is required to provide proof of full vaccination against COVID-19.

 
Position Requirements

Education

  • Bachelor’s degree in Social Work or Social Service Work with extensive experience working with parents of children 0-18yo; community development experience is an asset; or an acceptable equivalent combination of education and experience.

Professional Experience

  • Three to five year experience providing direct family support and community resources referral services to families relevant to vulnerable families with children, including for children with special needs.
  • Three to five years full-time experience providing supportive services to low income, multicultural families, and vulnerable families.
  • Experience and interest in working with marginalized and diverse clientele, such as low-income, high-needs, newcomer, and a variety of multicultural communities, presenting complex needs.

Key Competencies

  • Knowledge of child development and behaviour management.
  • Knowledge of local resources for families, children, including children with special needs, and their referral pathways.
  • Understanding of complex family dynamics and issues relevant to working with vulnerable, multi-risks parents/caregivers.
  • Ability to work independently and in a multi-disciplinary team environment.
  • Demonstrated ability to work in a multi-cultural, multi-disciplinary team environment.
  • Ability to work and establish trusting relationships with diverse families, including marginalized, low-income, multicultural, history of trauma.

Linguistic Profile

  • English: fluency speaking/writing/reading.
  • A second language (French and/or relevant to our client population) an asset.

Salary Range $26.27/hour - $30.90/hour
