The Richmond Behavioral Health Authority is seeking a Hybrid / Remote candidate for the role of REACH Coordinator. This position provides clinical and case coordination services to adults with Developmental Disabilities. This position is based from the person's home, however does require in person face to face visits and participation in an on call, mobile crisis response rotation at least 2 x a month. This position offers professional development opportunities, supervision towards licensure for eligible candidates, and along with the other benefits package offered by RBHA.

Job Duties

  • Maintains communication and relationships with community partners.
  • Ensures the coordination of support meetings and crisis plans for individuals served through REACH supports.
  • Participtes in on-call schedule, including mobile crisis response, approx 2 x a month.
  • Participates in required meetings with team members, supervisors, and clinical staff.
  • Coordinates individuals care with identified CSB/BHA Case Manager/Support Coordinator and applicable crisis responders.
  • Identifies and coordinates necessary training and technical assistance needs for providers, natural supports and other components of the system of care.
  • Conducts and prepares crisis assessments and plans, completes intakes as needed, and completes progress notes and related documentation in an Electronic Health Record system, in accordance with regulatory requirements.
  • Completes professional development throughout the year.
  • Travels to off-site appointments/meetings.
  • Participates in a multi-disciplinary treatment approach, including following up on recommendations, making service linkages, and presenting cases for feedback on a recurring schedule.
 
Position Requirements

Education and Experience

  • Master’s degree with coursework in counseling, social work, psychology or related field
  • Experience with (or the ability to demonstrate knowledge about) the Developmental Disability and population
  • Current Virginia license as an LPC, LCSW, or LMFT, LSATP or license eligible preferred

Special Requirements

  • Other Requirements: Incumbent must maintain a valid Virginia driver’s license throughout his/her period of employment and be willing to use own vehicle for travel at times. Travel within a 150 mile radius will be expected so that services can be provided throughout Region IV. Flexible work hours, to include evenings, weekends, and holidays, are required. Duties require entering a variety of home settings and living arrangements
  • Qualifying licensed and licensed eligible clinician staff will be eligible for a $4K sign-on bonus as defined by the terms and conditions of the approved sign-on bonus policy.

Starting Salary Range Negotiable depending on qualifications and experience.
