Description

Clinical Social Worker needed for full-time, temp to perm position in Chicago. This position is a Monday through Wednesday and Friday from 9 am to 4 pm working with outpatient adolescents and adults.

Job Purpose:

Provide social services and assistance to improve the social and psychological functioning of adults, children, and their families. Social Workers in this field assess their client's needs and aid to improve their situation. This often includes coordinating available services to assist a child or family.

Duties:

  • Uses appropriate tools to make assessment and evaluations; prepares summary and makes recommendations regarding program services; communicates information to treatment team; maintains knowledge of updated program changes; maintains knowledge of ASAM and DSM V patient placement criteria for appropriate placement and continuing care.

  • Assists with monthly and yearly audit of patient records by utilizing Quality Assurance Audit Tool to ensure compliance with local, state, and federal guidelines and conducts research, advocate for improved services.

  • Conducts individual, group, and family therapy, outreach, crisis intervention, to improve social rehabilitation. Teaches skills needed for everyday living and helps clients who face a disability, life threatening disease, or social problems, such as inadequate housing, unemployment, or substance abuse issue.

  • Develops treatment plans with clients, encouraging them to create and pursue goals for improving their lives.

  • Provides services to assist family members of those who suffer from addiction or other mental health issues.

  • Identifies potential conflicts, implements intervention techniques to prevent or intervene in potential crisis situations and assists families that have serious domestic conflicts.

  • Uses clinically appropriate professional terminology to complete forms and records, accurately and timely documenting member assessment, planning, and progress; maintain knowledge of laws and regulations surrounding confidentiality of alcohol and drug patient electronic medical record and paper charts.

  • Should be comfortable using a computer, electronic medical record (EMR) or Telehealth Program (ZOOM, V-SEE), its software, and all other office equipment.

  • Develops and maintains appropriate professional boundaries between self, staff, members, and collaterals and identifies issues related to ethics and boundaries, as identified in the Code of Ethics policy. Adheres to confidentiality requirements.

  • May be required to travel to schools, hospitals, or other treatment care facilities.

 
Position Requirements

Master's degree.

Two-year’s experience in a substance abuse treatment, correctional or other social service/mental health setting.

MSW, LSW or LCSW.
