The primary purpose of the job position is to plan, organize, and develop, the overall operation of the Social Services Department following current federal, state, and local standards, guidelines, and regulations, our established policies, and procedures, and as may be directed by the Administrator, to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.   Position Requirements BSW or MSW - Detail Oriented. Experience in skilled nursing preferred with an emphasis on discharge planning. Excellent customer service skills; dedication and commitment to providing quality healthcare services. Excellent ability to operate effectively and efficiently in an enthusiastic, patient-focused, work environment. Critical thinking and problem-solving abilities. Experience with Matrix EMR is preferred but not required. Good computer skills. Self-motivated. Able to meet firm deadlines for submission of Assessments, MDS, and Care Plans.
