Description

JOB PURPOSE:

The Case Manager is responsible for using sound professional judgment and best practices to ensure quality case management services are provided to the service population especially in crisis situations while maintaining compliance with established state and federal standards. Will provide information, guidance, service referral, and coordination for asylum seekers and other program participants as designated. The Case Manager monitors assigned caseload of clients and helps client address barriers to exit from city-supported housing; support completion of immigration paperwork, and connect clients with school aged children to NYC’s department of education. The Case Manager will meet with clients regularly.

 
Position Requirements

ESSENTIAL JOB RESPONSIBILITIES:

  • Provide client-centered, trauma-informed case management services to asylum seekers and other program participants.
  • Provide or access interpretation and translation services to/for program participants.
  • Cultivate new relationships with service providers and referral partners.
  • Coordinate and collaborate with key stakeholders and service providers to ensure that program participants are linked and offered services in a timely manner.
  • Meet regularly with clients and clearly document service provision through case notes and other program forms.
  • Enter all data into appropriate databases and keep client records up-to-date.
  • Provide support for immigration paperwork including work authorization, and connections to the City’s Department of Education for households with school-aged children expecting to be in the City for at least 5 days.
  • Responsible for protecting the confidentiality of any information or material obtained in the service with the organization to include but not limited to client names and information, services rendered to clients, donors names and gifts, internal and external investigations or results of any investigations, and financial information.
  • Provide culturally-competent casework services in the pursuit of supporting asylum seekers in the City’s care to resettle without needing to rely on city-provided housing.
  • Develop individualized exit strategy, defined as timelines and work plans for households to transition out of city shelter.
  • Refers clients to appropriate programs or other agencies based on clients’ needs and in accordance with their program plan.
  • Meet all deadlines required by program supervisor and partners.
  • Meet all federal and state regulatory guidelines and standards that are applicable to this position.
  • Maintain a caseload in accordance with agency policies and procedures contract standards.
  • Participate in workshops, seminars, education programs and other activities that promote professional growth and development.
  • Engage and involve client in the casework process.
  • Other duties as assigned.

ESSENTIAL QUALIFICATIONS:

EDUCATION: Bachelors or Undergraduate degree required, preferably in one of the following areas: Social Services, Human Services, Behavioral Science, Social Work, Psychology, Criminal Justice, Sociology or related field of study.

EXPERIENCE: Minimum 1-year related experience and/or training including experience working in asylum field in a social service setting. Preferred experience in working in a social or public service environment with migrants in a residential setting with case management experience.

ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the employer’s sole discretion.

LICENSES: Driver’s License with clear record.

VEHICLE: Must have daily use of a vehicle without prior notice.

OTHER: Must be available and willing to travel to various locations and with such frequency as the business need dictates. Must be 21 years or older. Bilingual English/Spanish required. Must not pose a direct threat or significant risk of substantial harm to the safety or health of himself/herself or others.

Salary/Wage $32/hr.
